Numerous providers have found our handy referral form helpful. Don’t forget to attach supporting documentation. Some carriers may also request a mental health letter.

Ask your team to send in a PA

Aren’t teammates the best? However your office handles this step, we know you’ll get it done. Some providers specify more in their PAs, but most importantly, please list specific areas of treatment.

North Bay takes it from there

Once we hear from the patient’s insurance company, we will reach out to them directly to book for consultation with one of our clinicians.
